Carolyn Alessi is the Project Director for Community Health and Well Being at Saint Francis Hospital and Medical center, a member of Trinity Health of New England. Under her portfolio of work are the Child Injury and Fatality Reduction program, the COVID-19 Outreach Initiative, Community Health Worker program, Enhanced Recovery After Surgery quality improvement program and the Hospital Violence Intervention program. She manages a grant portfolio of $1.5million. Her work focuses on supporting Black and Brown communities that experience social barriers to health. She works to improve coordination of clinical care with social care services to improve health outcomes in high risk patients of color. The work involves collecting “big” sets of data inclusive of social influencers of health, understanding how this data informs gaps in care and looks to create innovative approaches to health care and delivery of services working closely with community based organizations.
Carolyn received her Bachelor of Arts in Political Science from University of Massachusetts at Lowell, her MBA from Centenary University and has certifications in Healthcare Management and Women’s Leadership from Yale School Of Management Executive Education programs.
